Venlafaxine is an SNRI, it not only messes with serotonin but also norepinephrine. Made me anxious (likely because of the noradrenergic fuckery) and the withdrawal was a fucking BITCH.

If you're getting tachycardia to the point where you get symptoms (difficulty breathing, chest pain, dizziness, vision disturbances, pallor or anything like it), check it out some more. Plenty of benign reasons for tachycardia but then again, some aren't.
I'm not saying this to freak you out, but wearing a Holter monitor for a bit might be a good idea. It's sort of like a long-term EKG that helps diagnose intermittent issues.
Some breathing treatments push the same buttons adrenaline does. Kids can have pretty fast heartbeats without it being bad, your "max" heart rate exercising etc. famously goes down as you age. Must've been scary, though.
Mine were a bit less acute than most cases here. It doesn't rank up to the kind of emotional trauma other people ITT have been through (though I'm not a complete stranger to that, either) but mine was when I realized my health is going to prevent me from ever doing what I want to and getting my shot at a reasonably happy life.
The slow dread of realizing decades of miserable, exhausting, bitter, mostly hopeless, unappreciated effort is void, and has been a complete waste - realizing that things "working out" is not really on the table anymore, and neither's anything else, much: all you can do is keep existing. That is easily #1.
Or realizing friends and family didn't have my back the way I thought and might actually join my list of a zillion problems. That was pretty scary.

I think maybe the design isn't working ideally, it's relatively complicated stuff and "federation politics" makes it infinitely worse. I think it's going to be a hard sell for casual users.

It's arguably used wrongly on Discord, but not in a way that's radically different from how I already thought about "servers" in the sense of "something you connect to".
It seems more like a term they picked because it has that familiar sense. Otherwise I think there's a semi-official term, "guild", too.
I don't think it's about the term, "server" and "instance" both make sense to me. The issue is that the fediverse itself is pretty confusing.
The basics? Great: it's vaguely "IRC but persistent", all good.
But for starters it's hard to keep track of which instances actually exist - new ones pop up and old ones die at the drop of a hat.
Then there's differences in feature sets (lemmy vs kbin and whatever else) that happen to be ActivityPub compliant or whatever. kbin notably doesn't federate downvotes, for example. And all this software is still relatively immature.
Then there's the actual "who federates/defederates whom and why" debacle. This results in a lot of obvious and some less obvious visibility issues.
Then there's (other) individual instance politics.
Then there's the "meta" about all of this, which is getting confusing.
A couple of these will have parallels on e.g. Reddit - I assume this is the natural comparison to make and will keep being so for a while - like sub drama and the relationship between subs. But because the FV has this at the instance level, (and each instance has many "subs",) it's a whole level up in complexity.
Then there's how all of this makes for a pretty un-reddit-like experience - and Reddit is not the king of polish, either. While Reddit has duplicate subs, it doesn't have a design that almost automatically causes them to be created and distributed, across instances without actually correlating them afterwards. The end result is that subbing or blocking any one community will likely involve doing that manually on several instances, which is stupidly inconvenient. Also discoverability is much trickier which is worsened by the low activity.
My point is: call it what you want, but a) I don't think that's where the confusion is coming from - that's just the fediverse being confusing (and outright clunky in many regards), and b) obligatory XKCD "Standards".
No, there are two entire kinds of internal defibrillators, in addition to the external ones. One is just for surgery etc. when they have direct access. I think it would ultimately depend on a use case for that metric, otherwise you're putting the cart before the horse. There are many measurements and calculations you could come up with, but no obvious (to me, anyway) interpretation of "most discontinuous": something is either in one piece or not. If you needed a metric like this for a practical purpose, your specific needs would be a starting point for designing one. If it's more of a shower thought, you sort of have "too much freedom" to be able to define anything that's necessarily meaningful.
Simple examples would be just "number of 'discrete parts'", "minimal area needed to span all territories" and things like that. Maybe you're more interested in "total distance from all satellites to wherever the capital is" or something, in a different context. The point is they'd all tell you radically different things, so it's important to know which one to ask for.
You could argue that something like Hawaii and Alaska's distance from the rest of the US makes the US score highly.
You could argue that any number of island nations score highly because after all, most of e.g. the US is in one part.
You could argue e.g. Norway's territories near both poles make it pretty high-scoring too.
You could argue that for whatever reason, distribution of area and population matter, and so on.
